Handbook and Forms

Handbook - Members and parents/guardians are expected to  familiarize themselves with our policies and expectations.  Please note that there have been a few changes made.

Forms - The following forms need to be filled out and returned to one of the personnel: Heather Lyman (Symphony), Josie Anderson (Chamber), Josie Anderson (Wind Ensemble), or Jaquell Hubner (Preparatory Strings).

- Acceptance, Release, & Payment Form  - per family

- Medical Release Form  - per individual

Forms are due by the 2nd rehearsal.
